The Dream@50 Art Contest Submission Form may only be used by school administrators or teachers and after-school program directors or teachers. Only one art submission per school and after-school class is accepted. ALL contact information fields are required, unless otherwise noted, including phone and email. Be sure to check the Consent box at the bottom of this page after reading the Submission Consent Agreement.
If the submission is successful, the following notice will appear: Thank you. Your entry has been successful. If the submission is not successful, there will be an error message in red at the top of the submissions form page that will identify the issue. After rectifying the error please be sure to re-upload the art image. If there are any questions or problems, do not hesitate to email firstname.lastname@example.org.
